Liability: By accepting this agreement, I attest that the student(s) entered are in good physical condition and able to fully participate in the prescribed course of instruction. I understand that the school, the owners, operators, instructors or persons in charge of such establishment or employees will not be held liable for any injuries, loss or damages. Student(s) agree that all activities are taken at his/her own risk. I also authorize the school and its agents, successors, and assigns to photograph or film me and/or my children and/or use voice without restriction and utilize such photographs and/or voice transcriptions for any commercial purpose, including but not limited to the promotion and marketing of the school. I agree that there is no entitlement to receive any compensation whatsoever of any kind as a result of such use.
Health and Safety: I understand that diseases such as COVID-19 are highly contagious and that the CDC, State of California and San Mateo County Health Department have published protocols that members of the public must follow. I agree that all members of my family understand and will follow these published protocols when participating in in-person activities operated by Core Taekwondo. I understand that if any member of my family is experiencing symptoms including a fever, cough, or other COVID-19 symptoms, is believed to have been exposed to someone with a suspected case of COVID-19 or has been diagnosed with COVID-19, that we will not be permitted to attend in-person activities until cleared as non-contagious by State or Local health authorities. I further acknowledge that Core Taekwondo cannot guarantee that members of my family will not become exposed to or infected with COVID-19 when attending in-person activities as the result of actions, omissions, or negligence of my family and others, including, but not limited to, the owners, operators, instructors or persons in charge and their families.
